What is patella surgery?
Patella surgery addresses pathologies of the patellofemoral joint, that is, the articulation between the kneecap (patella) and the femur. It is mainly indicated in cases of recurrent dislocations, chronic instability or persistent pain linked to poor positioning or wear of the patella.
The goal of the procedure is to restore proper patellar trajectory, stabilize the joint and relieve pain, while preventing recurrence or premature wear of the cartilage.
When should surgery be considered ?
Dr. Coulin offers surgery in the following situations:
- Repeated luxations of the kneecap, despite rehabilitation.
- Patellofemoral instability with apprehension or sensation of slippage.
- Chronic pain resistant to medical treatments (physio, infiltrations).
- Patella-femoral dysplasia (joint malformation).
- Mechanical misalignment of the knee or patella abnormalities detected on imaging.
Before any surgery, a complete assessment is carried out : precise clinical examination, X-rays, MRI and sometimes a scanner to analyze the axis of the knee, the shape of the trochlea (femoral groove), the height of the patella and the mechanical constraints.
What does the intervention consist of ?
There is no single kneecap surgery; several procedures can be considered, depending on each situation :
- Refocusing of the kneecap,
- Reconstruction of the medial patellofemoral ligament, in case of instability,
- Lateral release in case of excessive tension,
- Bone correction if the limb axis is pathological,
- Gestures on the cartilage if an injury is associated (microfractures, cartilaginous graft).
The procedure is performed under local or general anesthesia, most often on an outpatient basis or with a short hospital stay.
What are the post-operative consequences ?
The after-effects vary depending on the procedure performed. After ligament reconstruction or patellar realignment, a protective splint may be worn for a few weeks. Walking with weight-bearing is generally possible quickly.
Rehabilitation is essential for restoring knee mobility, strengthening the quadriceps, improving movement awareness, and relearning proper movement. Follow-up is personalized, with regular check-ups to adjust the recovery protocol.
What results can you expect ?
The results are generally very satisfactory, especially when the surgical indication is clearly defined and rehabilitation is followed seriously. Patients note:
- Une disparition des épisodes de luxation.
- A reduction in pain.
- Un gain de stabilité et de confiance dans le genou.
- Une amélioration fonctionnelle durable.
Dr. Coulin pays particular attention to the prevention of recurrences and the long-term preservation of cartilage.
